About Asymptomatic Spread

Asymptomatic transmission is the spread of an infectious disease by individuals who show no symptoms. This phenomenon was central to the COVID 19 pandemic. SARS CoV 2 can be transmitted 1 to 2 days before symptom onset pre symptomatic and by individuals who never develop symptoms truly asymptomatic. One widely cited modelling study suggested that roughly half or more of COVID transmission is from pre symptomatic or asymptomatic individuals. This limited the effectiveness of containment based solely on symptom screening and isolation. In contrast, SARS 2003 had minimal asymptomatic transmission, which facilitated containment. Typhoid Mary is a historical example of asymptomatic carriage. HIV can be transmitted by asymptomatic individuals for years. Tuberculosis, hepatitis B, and chlamydia also have asymptomatic transmission phases. Asymptomatic transmission was cited by public health agencies as a rationale for population wide measures such as masking, ventilation, and vaccination. The concept challenged early COVID guidance that focused on symptomatic individuals.
