Clade X was a pandemic tabletop exercise hosted by the Johns Hopkins Center for Health Security on May 15, 2018. The exercise simulated a novel parainfluenza virus released deliberately. The fictional scenario projected more than 150 million deaths globally. Participants included former government officials playing the National Security Council. The exercise revealed that current pandemic response capabilities were severely inadequate. Key findings included insufficient vaccine manufacturing capacity, unclear decision making authority, inadequate medical countermeasure distribution, and lack of coordination between agencies. The exercise recommended strengthening the Biomedical Advanced Research and Development Authority BARDA, improving vaccine manufacturing infrastructure, and clarifying crisis communication. Clade X was part of a series of exercises including Dark Winter 2001, Atlantic Storm 2005, and Event 201 2019. These exercises consistently identified similar gaps in preparedness. Despite these warnings, many recommendations were not implemented before COVID 19. The exercise series shows that the risk of a severe pandemic and the gaps in preparedness were widely anticipated.
