Africa experienced fewer COVID cases and deaths than predicted by early models. The continent had reported roughly 12 million confirmed cases and about 250,000 deaths by 2024, far fewer per capita than Europe or the Americas. Several factors may explain the lower burden. Africas population is young, with a median age of approximately 19 years compared to 38 in the US. Cross reactive immunity from prior coronavirus exposure may have provided some protection. Outdoor lifestyles and lower urban density in many areas reduced transmission. Some researchers proposed that BCG vaccination provided non specific immune benefits, a hypothesis later trials did not support. Underreporting was significant due to limited testing capacity. Seroprevalence surveys suggested actual infections were far higher than confirmed cases. South Africa was the hardest hit country, experiencing five waves, including a devastating Delta wave and an Omicron wave in late 2021. Africa faced vaccine inequity, with less than 20 percent of the population vaccinated by mid 2022. The lower than expected mortality remains a subject of scientific investigation.
