Long COVID research accelerated after millions of people reported persistent symptoms following acute infection. The NIH launched the RECOVER initiative with 1.15 billion funding. Research identified multiple potential mechanisms including persistent viral reservoirs, autoimmune responses, microvascular damage, autonomic nervous system dysfunction, and mast cell activation. Imaging studies showed changes in brain volume, cardiac inflammation, and lung scarring in some patients. The WHO defines post COVID condition as symptoms that usually begin within 3 months of infection and last at least 2 months. Approximately 65 million people worldwide are estimated to have long COVID. Risk factors include female sex, severe acute infection, asthma, and EBV reactivation. Observational studies associate vaccination with a reduced risk of long COVID, and studies of whether antivirals reduce the risk have produced mixed results. Clinical management remains largely focused on symptoms. The economic impact is significant, with some estimates suggesting around 1 million Americans out of work due to long COVID. Some commentators have described long COVID as a mass disabling event.
