COVID 19 exposed extreme vulnerabilities in global medical supply chains. China produced around half of the worlds surgical masks before the pandemic and a significant portion of active pharmaceutical ingredients. When China locked down, global supply chains froze. India restricted exports of 26 pharmaceutical ingredients including paracetamol. US officials noted that a large majority of facilities producing active pharmaceutical ingredients for the US market were overseas, primarily China and India. Hospital drug shortages increased significantly during the pandemic. The Defense Production Act was invoked to force US companies to produce ventilators and PPE. After the pandemic, multiple countries announced reshoring of pharmaceutical production. The EU published a Pharmaceutical Strategy emphasizing supply chain resilience. However, reshoring is expensive and slow. The concentration of pharmaceutical manufacturing in a few countries remains a strategic vulnerability. The Biden administration commissioned a supply chain review that identified significant dependencies. The COVID supply chain crisis motivated broader discussions about strategic stockpiles and domestic manufacturing for essential goods.
